Why do we need your private information?
This information is required to manage your dental records and account, and to provide treatment that is appropriate for you in a safe manner. All information is regarded as confidential and will not be made available to any individual, entity or organisation without your consent, in keeping with the provisions of the Privacy Act 1993 and the NZDA code of practice on patient records. Please complete this form carefully, giving as much information as possible, even if something may seem unimportant to you. Your dentist will be happy to review your answers with you at your next visit and answer any questions you may have. Thank you for taking time to be comprehensive and accurate. Change of appointment
If you need to change the appointment, please make sure that you tell that in advance so that we can slot another patient in who may need to wait much longer if you don’t tell us that you can’t attend to the appointment. We ask you to let us know at least 48 hours in advance if you can’t come in for your appointment. Thank you for being considerate.
Fees
You can expect to pay NZ$50.00 for a routine consultation or full examination with two X-ray films. Hygienist services and treatment items are additional. After this initial diagnostic appointment, an estimate of fees will be presented to you with your treatment plan. A written estimate is readily available on request. Please feel free to discuss fees at any time with either dentist or receptionist. Investing in one’s oral health is money well spent when the long term benefits of good dental care and keeping one’s teeth healthy for life are considered
Payment for services
All services are required to be paid in full at the time of treatment. We accept Cash, Cheque, EFTPOS, Visa, MasterCard, Activa and Bankcard.

Our practice
Our spacious, custom built practice consists of an open plan entrance and a friendly reception area and front office. The Front Office Coordiantors answer your calls and manage all the daily activities that happen in a busy practice. We have five treatment rooms, two fully equipped instrument sterilization room and a disability-friendly bathroom. You may wish to touch up your makeup after treatment because we know that you want to feel your best when leaving the practice and go about your day.
If you have any questions before you go through to the treatment room or after, please feel free to talk to my assistant Clare or any of the friendly staff. All have knowledge about dental health and treatments. We believe the more you know about your dental condition the better you are able to make the right choices for you and your health.
It is our duty to inform and educate our patients about their dental status and available treatment options. We always support our patients’ choice of treatment on condition that we are sure they understand the consequences of their decision.
Parking
We have disabled parking in the parking area behind the premises. This is accessed down the driveway to the right of the building.
